Adoptive parent(s) who have a child placed for legal adoption by an authorized placement agency and cannot obtain the child’s SSN after reasonable attempts must file Form W‑7A.
The form collects the adoptive parent’s address (Line 2), the child’s adoptive and birth names (Lines 3a‑3b), date of birth, sex and place of birth (Line 4), and the name, address and placement date of the authorized placement agency (Line 5), plus a signature.
You must complete Form W‑7A at least 4 to 8 weeks before you will need an ATIN.
Mail the signed Form W‑7A and attachments to Department of the Treasury, Internal Revenue Service, Stop 6182, Austin, TX 73301‑0066.
First, fill out the required lines: Line 2 with your address, Lines 3a‑3b with the child’s adoptive and birth names, Line 4 with the child’s birth date, sex and place of birth, and Line 5 with the authorized placement agency’s name, address and placement date. Attach signed and dated proof that the child was placed with you for legal adoption. Both adoptive parents sign and date the form. Mail the completed form and attachments to the IRS address listed.
