Form 8918 lets a material advisor report a reportable transaction to the IRS and obtain a transaction number. The advisor must provide basic contact details, describe the transaction, and sign the form.
Every material advisor – an individual, trust, estate, partnership, or corporation – to a reportable transaction must file IRS Form 8918.
The form collects the sender’s name, title, phone number, street address, the material advisor’s name, the filing date, number of pages faxed, and the Internal Revenue Code sections used for the transaction.
If you cannot fax electronically, mail the completed Form 8918 to Internal Revenue Service, OTSA Mail Stop 4915, 1973 Rulon White Blvd., Ogden, UT 84201. Fax may be used within a 100‑page limit.
Enter the sender’s contact information, the material advisor’s name, the filing date, and the number of pages faxed in the top section. List the Internal Revenue Code sections that generate the tax benefit on line 12 of page 2. Attach any required statements and sign under penalties of perjury. Fax the form (max 100 pages) or mail it to the OTSA address.
